Outcome of Renal Transplant gets compromised if patient is over-weight

Lady from Bangladesh deemed fit for Renal Transplant after undergoing Bariatric Surgery at Rabindranath Tagore International Institute of Cardiac Sciences

Kolkata, 2nd December, 2015: Rabindranath Tagore International Institute of Cardiac Sciences (RTIICS) announced the successful weight loss surgery of a 53 years old lady from Bangladesh who was initially declared unfit for renal transplant. Mrs. Razia Begum was presented at RTIICS with chronic kidney disease (Stage 5) and was on maintenance haemodialysis at that time. A renal transplant was recommended as the only option to save her life. During optimization for Renal Transplant, she was deemed unfit for transplant due to her morbid obesity by the transplant team. Therefore she was referred to the department of Bariatric Surgery at RTIICS and underwent a weight-loss surgery. Post the surgery, the patient lost 15 kilos within 2 weeks and has now been declared fit for a renal transplant.

As the dietary restriction and medication to lose weight did not work, the patient was referred to the Bariatric Surgery department for a formal assessment. In her assessment it was found that her BMI was as high as 38 kg/m2, while the optimal BMI is within the range of 21 kg/m2 to 25 kg/m2. Efforts to lose weight through dietary restrictions and medications had failed. After evaluation by the team, it was decided that the patient would benefit from a Laparoscopic Sleeve Gastrectomy (LSG), which would aid in her weight reduction. She was then counseled for bariatric surgery, which included psychological counseling along with a review of her medical history. After being on strict diet and exercise, she was admitted a day prior to the surgery and dialysis was done.

Commenting on this case, Dr. Shubhayu Banerjee, Consultant – Advanced Laparoscopic, Gastro Intestinal and Bariatric Surgeon said, “Renal transplantation is a life saving option for most patients with end‐stage renal disease. However, availability of kidney donor is limited and therefore, selection of patients who are likely to have a positive outcome remains an important issue. Basis this, obesity is often considered a hindrance to successful outcome especially those with morbid obesity. To add on, morbid obesity, has added risks as in case of this patient, who was also suffering from asthma and hypertension”. Commenting on why weight-loss is important for a renal transplant patient, he said, “There is not much study on this. But the chances of transplant rejection and related complications are quite high. The outcome of renal transplant is compromised if the patient is overweight. Hence this patient underwent a bariatric procedure to get her renal transplant done”.
